Ask your friend, what his small 3 has about him that can be exposed? Chat history, sex vid or what?
She knows where he stays, his work place or what?
Anyway, no matter what, I assumed your friend is married thus has more at stake, he should assume the small 3 can cause more damages.
He needs to think how to manage her and gradually let her grows out of him, ie no longer like him to want to break up with him instead. He can start by doing something that she dislikes, eg praising all ladies appearance or figures.
The last thing he should do is to ask for a break up now. She will become ballistic.

There's no right or wrong, only for themselves to decide. In case your friend has made some promises or has led her to think there's a future for them.
Meals and hotels are not part of the deal lah. Even if he has been showering her with gifts. All these are not the same like paying a fee that both agree as a commercial transaction.

Sharing my experience...

Was kinda in a hanky panky relationship with a colleague during my low-time while dealing with my divorce. Maybe was weak mentally then and got involved with the colleague (let's just name her SW). We would have sex and she would also gladly swallow me when she had her period. However I can feel alot of colleagues eyes on us and I things had to stop when she told me she is considering divorcing her husband to be with me. She had also been unhappy with her married life. With my current mess, I definitely don't need another added mess. So I told her we need to stop and she need to to counselling for sake of her children.
We did have a one-off where she came to my house to drink and she blew me. Sometimes when she walked past my desk I can still see her stealing peeps at me. I'm sure if I'm gonna grab her, she will not hesitate to blow me anytime.

Another colleague that I'm currently "eyeing" is a new girl (lets call her CK) with the good girl vibe. And I recently noticed that she has at least a big C/ D cup from side view. Sometimes when our eyes met, she will give that impish grin or smile. Been thinking of asking her out alone for lunch, but being that I'm newly remarried, I doing my best to push that urge away.
