Most of the time, the easy way to tell is if card is needed to access lift, no card, means they don't really care too much.
Also, as long as the person comes in with the registered guest, the staff cannot really tell if thats an invited guest or an already registered guest.
Unless its a provocatively dressed lady that catches the eye

Timing is another problem, during the peak hour and when hotel is busy. No one cares about who come into witch room and massing up with who. But during the mid night, a couple walk into the hotel sure will catch eyes.
