Yes, they will ask. Just tell them you have discomforts hindering satisfactions when having sex and hygiene wise there's bad odour also tend to get rashes and itchiness when not properly washed. Of course your appproach must be nice and courteous, not rude and demanding as though you know better than them, it will piss them off...
When they write you the referral letter, most of the details are already in it so hospital usually will not ask anymore. Even if they ask you already knows what to say.
