Why do some ladies have a protruding anus, somewhat like a lump of mass instead of a natural hole? Any kind samsters can shed light?

She probably has piles (haemorrhoids) for a long time and some develop these lump of skin or mass just outside the anus due to prolonged time spent in the toilet suffering from constipation or diarrhoea. May even bleed excessively at times so anal sex is a no no as it might be really painful.
